What is Care Management:

Care management in Northumberland is an approach to providing planned services to service users and carers through the close joint working relationship between Health and Social Services.

Any adult needing care and support because of a disability or special need is offered an assessment by a care manager. All care managers follow a common approach to assessing peoples problems, agreeing care plans and monitoring and reviewing the care plan in line with the authorities standards and guidelines.

What is a Care Manager:

When someone is ill or disabled, they may need help to solve a variety of problems. A care manager is someone who can help you to find out about all the options available. She/He can give you advice about what might suit you best, and can arrange many kinds of help directly.

Who is my Care Manager:

Care managers can come from a number of different professions. For instance, they may be social workers, community nurses, or occupational therapists. Many of them do other kinds of work as well, but they all work in the same way when they are acting as care managers.
