When someone is ill or disabled, they may need help to solve a variety of problems. A care manager is someone who can help you to find out about all the options available. She/He can give you advice about what might suit you best, and can arrange many kinds of help directly.
Care managers can come from a number of different professions. For instance, they may be social workers, community nurses, or occupational therapists. Many of them do other kinds of work as well, but they all work in the same way when they are acting as care managers.
