Information Regarding the Summary Care Record
All patients will be receiving a letter regarding their Summary Care Record (SCR). It is very important that you read this carefully as it contains information relating to your medical records.
You will automatically be given an SCR unless you OPT-OUT. To Opt-out you need to complete a form. This can either be collected from Reception or you can download it here, complete it and return to The Practice Manager at Wellway Medical. This information will be added to your health record so an SCR will NOT be uploaded for you.
Any patients who have already opted-out do not need to do this again. However, if you have already opted-out and now wish to have an SCR please inform Reception.
