The Practice and Community Nurses in the Primary Health Care Team have worked very closely together over the years and this has been actively encouraged by the Partners. However, to improve services further, regular reviews are undertaken to ensure a full skill mix is established in order to provide patients with a completely integrated nursing team. This essentially means that the Practice and Community Nurses carry out the same roles and responsibilities. The members of the team are:
I worked as a Community Sister at Wellway for 20 years. My professional interest is diabetes. I developed my role as lead care manager, helping to integrate health and social care and had a special interest in the development of nurse triage. I have recently changed from my Community role to become a Practice Nurse for Wellway Medical Group, overseeing the nursing team.
Practice Nurse. I have previous experience in medicine and coronary care. My specialist interests within the practice are blood pressures and home monitoring plus the Wellway Travel clinic.
Practice Nurse. My professional interests are in Respiratory medicine. I am a member of the respiratory nurse interest group and attend regular updates in respiratory disease. I enjoy advising people about asthma, chronic airways disease and manage smoking cessation clinics. My other professional interest is cervical cytology and I am responsible for cytology audit and peer review of the practice nursing staff within the practice.
Practice Nurse. I am a qualified Family Planning Nurse and a member of the National Association of Nurses for Contraception and Sexual Health. I also have an interest in helping women with Menopausal problems and advising on Osteoporosis and HRT.
I am a Community Nurse. My special interests include treatment of leg ulcers and wound care, monitoring of patients on drug therapies for rheumatoid arthritis, psoriasis, irritable bowel and incontinence, especially in women.
As part of the integrated nursing team I work in the Surgery and also out in the community. My duties in the surgery are urine testing, blood clinics, helping nursing sisters with Diabetic/hypertensive clinics, assisting the doctors with minor surgery, ordering information leaflets and helping the Health Visitor with the baby clinic. My Community duties include caring for the terminally ill patients, weekly B.M checking for house bound patients, bathing and minor dressing and ulcer care.
I have been a Community Nursing Sister at Lynemouth Surgery for 20 years. My interests include preventative medicine, wound care and enabling patients to stay in their home environment whatever their problems.
I have been a Community Nurse for 10 years, 8 years of which were spent at Lynemouth surgery. My special interest is care in the community and I intend to develop my skills in the diabetes. I also provide support to the nursing team by undertaking treatment room sessions at all three Wellway sites.
I trained at University of Northumbria between 1991-94. Subsequently I have worked in Neurosurgery and trauma at Newcastle General Hospital until joining Wellway in January 2003 as a Practice Nurse.
I have worked in Community nursing for 11 years, covering the Out of Hours service as well as working in many local surgeries. I have been at Wellway 5 years and am experienced in most aspects of Community nursing. I am also experienced in a large amount of practice nursing. I have completed several courses in palliative care and intend to take this further to diploma level. I also have an interest in wound management.
